A covering letter from Bishop Alan Clark is included that provides some context for these sections within the rest of chapter 5. The first record of publication of Fitzpatrick's book appears to be in 1993 (Cambridge University Press) with the slightly revised title In Breaking of Bread: The Eucharist and Ritual.
Further research under the pseudonym G. Egner reveals other contemporaneous essays on transubstantiation:
